Survey for Medical Centres

Below is a survey for the 18 medical centres who have implemented the ManageMyHealth(TM) (MMH) patient portal programme within Hauraki PHO. 

 

The aim of the survey is to understand:

1.  What are the medical centres' perceptions of the MMH patient portals

2.  What are the perceptions of their patients' perceptions of the MMH patient portals

 

The objectives of the survey:

1.  The survey will be open April 11th to May 9th.

2.  I am requesting one survey from each position specific role at the 18 medical centres complete this survey.  For example, one Practice Manager, one GP, one Nurse Practitioner, one Nurse, one Receptionist, etc.

My promise to participants:  

1.  Data regarding patient uptake is available through this website.  To access your centre's data regarding the MMH patient portal click on the Patient Portal Implementation tab at the top of this webpage.

2.  I will ensure all centres have access to the results of the survey and results of the evaluation through this website.

Why is your participation so important?  Program evaluations of eHealth initiatives are critical to understanding:

1.  Where we are at with implementing the program and why

2. What was the program supposed to deliver and is it doing just that

3.  Where do we want to go from here  

4.  What are the lessons learned
